The link was working when this was origionally posted. I recieved my package from them. The link took you directly to the form to fill out so if they have stopped offering it odds are that they have disabled that page which is why you are getting the error that the page cannot be found. Their main site is still up and running with no problems so that is what I am guessing happened with this.
