I did the survey, but there was NOTHING about a $10 gift card. It didn't even ask for my address. Waste of time for me.

They probably send it to your email address for the chapters website.. or will send u a from to submit your address when the survey is over .... and if you read the page that the link takes you to, before u start the survey it clearly says:

"Thank you for taking a moment to answer this short survey about overactive bladder. It should take you no more than 5 minutes to complete. You will receive a $10.00 Chapters gift certificate in the mail after you complete the survey."
