    Residents of Ontario can take part in the Ontario Health Study and receive a $10 Gift Card for your contribution. Choose from Bet Buy, Canadian Tire, Cineplex Entertainment, Esso, HBC, Home Depot, iTunes, Sears, Shoppers Drug Mart, Sobeys or Walmart.

    This offer is limited to the first 33, 431 participants who register and complete the survey by March 15, 2012. Gift card selection must be completed by March 22, 2012.

    I couldn't even get to the survey cuz their page is all messed up and won't continue on. What a crock of crap and waste o time that was!

    FYI This is a very legitimate study done by the folks at MARS (health research) - if you don't know your DIN's it is not a problem and you are not required to give your HealthCard number. If you are interested in further surveys, you will be asked to be put on a list.

    PERFECT DAY FOR A GC, it is my birthday! (had to change the date on the survey LOL)

    Hi Rainbows - they were actually excellent for me - less than 2 weeks. Maybe it depends on what card? I got the SDM one.

    Veggy, I took the Shoppers Drug Mart too, and I did the survey like on March 13 and also did the gift card selection that day. And on 19th March, I got a mail saying they send my gift card. Do not when I will get it
