THE “A Slice of Life” SWEEPSTAKES: How To Enter: Beginning April 7, 2008 at 12:01 PM (ET) through June 16, 2008 at 12:00 PM (ET), go to omagazine.info/sliceoflife, and complete and submit the entry form pursuant to the on-screen instructions. Winner Selection: Winner will be selected in a random drawing from among all eligible entries received on or about July 1, 2008. Drawing will be conducted by O, The Oprah Magazine whose decisions are final. Odds of winning will depend upon the total number of eligible entries received. Canadian winners will be required to correctly answer a mathematical skill testing question as a condition of receiving the prize. Prizes & Approximate Retail Value: One (1) Grand Prize winner will win a gift certificate to a local spa including a day of spa services for two. Total ARV for all prizes awarded: $2,000. Winner will be notified via phone, e-mail or postal mail, at Sponsor’s discretion, on or about July 15, 2008. Winners’ List: For winner’s name, send a separate self-addressed, stamped envelope to “A Slice of Life” Sweepstakes” c/o O, The Oprah Magazine, 300 West 57th Street, 35th Floor, New York, NY 10019 by July 1, 2008.

2. ENTRIES: Limit (1) one entry per household for “A Slice of Life” sweepstakes (referred to as the “Promotion”). Multiple entries from the same household will be disqualified. Entries using macro, robotic, script or other forms of automatic entry will be disqualified. Entries become the property of the Sponsor and will not be returned. No mechanically reproduced entries permitted. Proof of submission does not constitute proof of receipt. If there is a dispute as to the identity of an online entrant, the prize will be awarded to the authorized account holder of the email address. The “authorized account holder” is defined as the natural person to whom the email address is assigned by an internet service provider, online service provider or other organization (e.g., business, educational institution, etc.) that is responsible for assigning email addresses for the domain associated with the submitted email address.

3. ELIGIBILITY: Open to legal residents of the 50 United States and the District of Columbia and Canada who have reached the age of majority in their state or province of residence at time of entry. Void in Puerto Rico, the Province of Quebec and where prohibited by law. Employees of Sponsor, its parents, affiliates and subsidiaries, participating advertising and promotion agencies, and prize suppliers (and members of their immediate family and/or those living in the same of household of each such employee) are not eligible.
