You only have to complete a very short survey and then when you fill in the address fields, be sure to enter your city, province and postal code in the city box, then in the zip box enter 11111. I did this and it works, then it asks you at the end of the survey if you'd like to receive a Canon Water bottle. Hope it comes.

its only for US. The postal code doesnt work with canadian ones.. =/

it will work if you put 11111 for the zip code and put your postal code in the city line as well as put your province. it worked for me, try it.
